This form is to be used when a caregiver adult wishes to disenroll a student from one member school for enrollment in another member school.  The form must be completed in its entirety before the request will be considered.

Notice of Interscholastic Athletic Participation: Per Board of Education policy 3620, a student transferring from one member school to another member school within Cherokee County Schools must sit out 365-days before being eligible to participate in any interscholastic sport.
